There is no additional charge for us to complete health forms as part of your check up visit.
An administrative fee is charged to complete health forms outside of an exam.
Please submit the form in the patient portal.
Daycare forms - $5
School & Sport Clearance - $25
Government Programs - $30
Physician Letter - $50
Medical Clearance
If you need to be cleared for a medical or dental procedure or surgery in which insurance is involved, your surgeon is required to request the consultation in writing.
Please specify that you need a consultation exam to clear you for a medical procedure when you schedule your visit and bring the request, or confirm our office has recieved it prior to your appointment.
We will provide a report including the results from any tests to your surgeon.
